What to Ask
Specific questions provide more specific answers. It’s best to keep the survey short and to the point. Most likely, there is a customer service issue. Especially when all else within our system is on track. Something we do not see blocks the road. And the goal here is to identify the roadblock.
Consider a check list or number system. Where a patient can circle a number of check a box. Number the rate of a specific service or experience from 1 – 5 or circle a word. Service is exceptional, good, needs improvement, poor, n/a. Something like this.
