Full mouth debridement update and major change for 2023. It’s now six months into the code update. The ADA heard the dental community’s cries for help on this one.
The procedure code D4355 was originally defined by the ADA was a preliminary procedure. One that had to be performed before an oral exam could be completed.
Why The Confusion?
The confusion came through the wording of the original and subsequent definition of D4355. The code came into effect in 1994.
Originally, the language stated that that procedure was performed to remove subgingival and supragingival plaque and calculus that obstructs the ability to perform a periodic exam.

Dental professionals argue that an oral exam and diagnosis are performed to establish the need for a full mouth debridement.
And the removal of the calculus and plaque then allow for a more thorough evaluation and treatment plan.

A Comprehensive Oral Exam may now be performed on the same day as the D4355. As the language and wording have been changed by the ADA.
The language now states that the full mouth debridement procedure is performed to enable a comprehensive oral periodontal evaluation and diagnosis on a subsequent visit.


Consider the Comprehensive Perio Evaluation
Depending on the risk factors and suspected perio involvement, the D0180 might be the next procedure to schedule.
The Comprehensive Periodontal Evaluation does require full periodontal charting as well as other specifications.

What about x-rays? You may want to consider these at the subsequent visit following the debridement procedure.
Perhaps stick with photos at the debridement appointment. The photos will support the dentist’s decision and treatment planning. Plus will show great before and after images.
